Injury Information
Online injury reporting - Mandatory for all injuries sustained
M-F-13 Sport Injury Report Form - Submitting an injury report opens a file with the insurance company for one year, without this the member cannot submit any claims for medical expenses. M-F-13 form is the basic injury report which can be sent to the ORA office by email or fax.
Injury Report - Physician form (insurance purposes) - Submitting an injury report opens a file with the insurance company for one year, without this the member cannot submit any claims for medical expenses. M-F-14, is the Physician Statement, which is required if the member is going to submit a claim for medical expenses. This form needs to be sent directly to the ORA office by mail.
